Submitting the "Book a Consultation" form is a request for an appointment, not a confirmed booking. A member of our team will contact you to confirm availability, scheduling, and any applicable consultation fee. We reserve the right to decline, reschedule, or cancel a requested consultation at our discretion, including where the information provided is incomplete, where the requested treatment is unsuitable based on a preliminary review, or due to clinic availability.
You agree to provide accurate and complete information when submitting the form. Please do not include full medical records, government ID numbers, insurance details, or payment information in the free-text fields of the form.
Browsing the Site or submitting a booking request does not, on its own, create a doctor-patient relationship between you and Desmoderm or any of its practitioners. A doctor-patient relationship, and any diagnosis, treatment plan, or informed consent, is established only through an in-person (or, where offered, a properly conducted teleconsultation) evaluation conducted in accordance with applicable Indian medical council regulations and, where relevant, the Telemedicine Practice Guidelines, 2020.
Information on the Site, including any description of treatments, expected outcomes, or before/after examples, is general in nature and may not reflect your individual circumstances. The Site and its content — including text, graphics, logos, images, and layout — are owned by or licensed to Desmoderm and are protected under applicable Indian intellectual property laws, including the Copyright Act, 1957 and the Trade Marks Act, 1999. Consultation and treatment fees, where applicable, will be communicated to you at the time of scheduling or at the clinic. Cancellation, rescheduling, and refund terms for paid consultations or treatments will be as communicated to you at the time of booking or as displayed at the clinic, and, unless stated otherwise, fees already paid for services rendered are non-refundable.
The Site and its content are provided on an "as is" and "as available" basis, without warranties of any kind, whether express or implied, including implied warranties of accuracy, completeness, merchantability, fitness for a particular purpose, or non-infringement, to the extent permitted under applicable Indian law. To the maximum extent permitted under applicable Indian law, Desmoderm and its practitioners, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of data, revenue, or goodwill, arising out of or in connection with your use of, or inability to use, the Site. You agree to indemnify and hold harmless Desmoderm and its practitioners, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable legal fees) arising out of your breach of these Terms or your misuse of the Site.
Our collection and use of your personal information, including Sensitive Personal Data or Information under the Information Technology (Reasonable Security Practices and Procedures and Sensitive Personal Data or Information) Rules, 2011, and the Digital Personal Data Protection Act, 2023, is described in our Privacy Policy, which forms part of these Terms.
